§ 13-9. Application for license; required information.

Applications for licenses under this chapter must be submitted to the tax collector, in duplicate, on forms furnished by that office. For each business location, the owner thereof must furnish financial data, record of amusement devices and music machines, and such other information as may be required by the collector in the enforcement of this chapter and may be held liable for the taxes due thereon, instead of the operator or owner of such machines, at the preference of the collector.
