§ 13-32. Application for license.
Every veterans, charitable, educational, religious, or fraternal organization, civic or service club, which possesses the appropriate nonprofit designation issued by the Federal Internal Revenue Service and who desire to operate a raffle or bingo game or to sell shares, tickets or rights to participate in such games, or to conduct such games, or to sell pull tabs, shall apply to the Ouachita Parish police jury for a license. Each organization desiring a charitable raffles and/or bingo license or pull tab license from the Parish of Ouachita shall submit an application which shall contain the following information, to wit:
(1)
Name;
(2)
Address;
(3)
Copy of charter of incorporation;
(4)
Names and addresses of officers;
(5)
Copy of the Section 503 exemption under the Federal Internal Revenue Code;
(6)
Description of the specific kind of game or games of chance to be held, including the place, date and time of such games of chance;
(7)
Anticipated items of expense to be incurred;
(8)
Names and addresses to whom the expenses shall be paid;
(9)
Purposes for which said games shall be held;
(10)
Specific purpose for which the entire net proceeds of such games are to be devoted;
(11)
Statement that no commission, salary, compensation, reward or recompense shall be paid to any person for holding, operating, or conducting or assisting in the holding, operation or conduction of such games of chance;
(12)
Description of prizes to be offered;
(13)
Copy of a license issued by the Louisiana State Charitable Gaming Division.
